Forms and Other Resources

In order to participate in activities, students are expected to abide by the district’s code of conduct in order to remain eligible. A copy of the code of conduct may be downloaded here:

Students must submit a health physical form in order to participate in athletics or physical education programs. In addition, students who participate in athletics and cheerleading must review the concussion fact sheet and sign an acknowledgement that both they and their parents have reviewed the information. Des Moines Public Schools high school athletics compete in the Central Iowa Metropolitan League (CIML) and are sanctioned by the Iowa High School Athletic Association and the Iowa Girls High School Athletic Union. There web sites have additional information about high school athletics in Iowa:
