Registration & Payment

Registration
Sessions are open to all interested teachers.  Registration will begin on Tuesday, February 21, 2012 and will remain open until July 20, 2012.

To register, download and complete the AP Workshop Registration Form, and submit payment as described below. (NOTE: the registration form is a fillable PDF.)

Payment
Payment must be received with your registration form.  If paying with a purchase order, actual payment must be received by the first day of the workshop, July 30, 2012.

  • If paying by purchase order, an electronic copy of your completed registration form and purchase order can be submitted via email to Brooke Jacobson at brooke.jacobson@dmps.k12.ia.us.  Registration forms and purchase orders may also be faxed to Brooke’s attention at (515) 242-7579.
  • If paying by check, your registration form can be printed and mailed with your check to Des Moines Public Schools, Attn: Brooke Jacobson, 901 Walnut St., Des Moines, IA  50309.  Please make your check payable to Des Moines Public Schools.

You will receive a confirmation email once your registration materials are received.

Cancellation Policy
All cancellations must be submitted in writing.  Cancellations received in writing by June 29, 2012, will receive a full refund. Cancellations made after June 29 will be subject to a $100 cancellation fee.  No-shows will be charged the full tuition amount.  Des Moines Public Schools reserves the right to cancel a session due to viability.  Registrants will be notified no later than July 23, 2012, if a cancellation is made.
